Afton in their bed is a close second, IMHO. and letting her take the blame for Mickey's accident, except I'm a little fuzzy on how that all went down at this point.

I wouldn't say CHEATING with Kimberly was the worst thing JR ever did to SE, but I would say setting SE up to catch them in bed together was the worst thing he ever did to her. Sure, it didn't work out the way he had planned, but he was creating the EXACT scene that happened with Holly and he knew that almost destroyed her, got a man killed, and ultimately led to the fire at South Fork. That's a new low, even for JR.
Wasn't it particularly cruel that JR killed Nicholas Pearce (in self-defense)? After all, we learned in the series finale that he would have been Sue Ellen's great love if JR had never existed.
The fact that JR kidnapped John Ross towards the end of Season 11 wasn't exactly nice, either.
I consider the forced commitment to the clinic and the affair with Sue Ellen's sister to be the worst aspects. The only thing missing would have been for JR to make a move on Patricia as well.
